The applicants shall write to the International Exchange Office of Changsha University of Science & Technology for the “Physical Examination Record for Foreigners” which shall be completed and then with photocopies of passport and some other documents mailed or E-mailed back to the university. The university will send the qualified applicants a Visa Application Form (JW202 Form) and CUST Admission Notice. The applicants shall apply for the students (x) visa by presenting these two documents at the Chinese embassy or consulate.

A. The following documents shall be provided by professional and undergraduate students:

1. passport(photocopy)

2. Physical Examination Record for Foreigners (photocopy)

3. Certificate of the highest academic degree earned (photocopy)

4. High school academic and graduate records (if have graduate exam)(original)

5. HSK exam record (original)
